Plan Of Care

The Plan of Care that the wraparound team creates with the family is the cornerstone of the work we do. The focus of this experiential training is developing skills for working with the family to identify core needs, produce effective strategies, and utilize strengths and resources. During the session, attendees will formulate a complete Plan of Care and evaluate it by county standards. Topics include:

• Understanding the ISC process and its timelines
• Defining Life Domains
• Reviewing the Plan of Care template
• Creating a Mission Statement
• Developing effective strategies to meet identified needs
• Improving Plan of Care documentation and reviews
• Improving outcomes for children and families

Family Safety Crisis Plan

Guiding families to an effective crisis planning process is essential to improving safety outcomes and providing good wraparound practice. Working with families to recognize warning signals, proactively plan for stabilization and manage crisis in an effective manner can be a challenging experience. This training provides attendees with an opportunity to create a Family Safety and Crisis Plan, measure it by county standards, and demonstrate its effectiveness through team work and role play. Topics include:

• Understanding the difference between safety/crisis and every day life
• Identifying triggers that lead to crisis
• Communicating with families about safety and crisis
• Creating effective plans with families
• Improving Family Safety and Crisis Plan documentation
• Improving safety outcomes for our families
